1. SPONSOR

The Nerve Reviver Sweepstakes (“Sweepstakes”) is sponsored by:

2. ELIGIBILITY

This Sweepstakes is open only to legal residents of the United States who are 18 years of age or older at the time of entry. Employees of the Sponsor, its affiliates, subsidiaries, advertising and promotion agencies, and their immediate family members are not eligible to participate.

Void where prohibited by law.

3. SWEEPSTAKES PERIOD

The Sweepstakes begins on August 15, 2025 and ends when total ticket purchases reach $6,500 or at 11:59 PM ET on September 15, 2025, whichever occurs first (“Sweepstakes Period”).

Sponsor reserves the right to close the Sweepstakes earlier if all entries are received before the deadline.

4. HOW TO ENTER

a) Paid Entry

Participants may enter by purchasing a sweepstakes ticket at the following prices:

1 ticket for $10

2 tickets for $20

3 tickets for $25

Participants may purchase entries for themselves or for a loved one. There is no limit on the number of tickets a participant may purchase.

b) Free Entry (No Purchase Necessary)

To enter without making a purchase, participants may submit a handwritten entry on a 3x5 inch card with the participant’s full name, address, phone number, and email address. Mail it in a stamped envelope to:

Mail-in entries must be postmarked by September 10, 2025 and received by September 15, 2025 to be eligible. Each mail-in entry will count as one (1) entry. Limit one mail-in entry per person per day.

5. PRIZE

One (1) Grand Prize will be awarded:

The winner will receive the Nerve Reviver program, including access to Dr. White’s at-home neuropathy care protocol.

Approximate Retail Value (“ARV”): $6,500

Includes shipping within the United States.

The prize is non-transferable, has no cash value, and may not be exchanged for any other product or service.

6. WINNER SELECTION & NOTIFICATION

The potential winner will be selected randomly from all eligible entries after the Sweepstakes Period ends. The drawing will be conducted within 7 days of closing the Sweepstakes.

The winner will be notified by email and/or phone. If the winner cannot be contacted within 48 hours, refuses the prize, or is found ineligible, another winner will be selected.

7. ODDS OF WINNING

Odds of winning depend on the total number of eligible entries received during the Sweepstakes Period, including both paid and free entries.

8. PUBLICITY RELEASE

By entering, the winner grants the Sponsor permission to use their name, city/state, photo, and likeness for promotional purposes without additional compensation, unless prohibited by law.

9. GENERAL CONDITIONS

Sponsor reserves the right to cancel, suspend, or modify the Sweepstakes for any reason, including fraud or technical issues.

Sponsor is not responsible for lost, late, misdirected, damaged, illegible, or incomplete entries.

Entrants agree to release and hold harmless the Sponsor and all related entities from any liability arising from participation or acceptance of the prize.

10. PRIVACY

12. GOVERNING LAW

This Sweepstakes is governed by the laws of the Commonwealth of Virginia, without regard to its conflict of law principles.
